Please find below important information that must be completed/updated by families prior to the new academic year. Please click the links throughout to open up adjoining forms.
1. Annual Bus Form CLICK HERE
Who should fill out this form?
- ALL students who will ride a bus in 2022-2023. Transportation forms must be completed for each new school year.

2. Online Registration Update Click Here and Residency Verification Materials
Who should fill out this online form?
- ALL new students. ANY returning students entering grades K, 3, 6 or 9 should email or drop off updated residency forms directly to their school's main office. See below for your school secretary's contact information.
Why do I need to Re-register my child if they already attend Watertown Public Schools?
- Only Parents/Guardians of all new students entering the Watertown Public Schools are required to complete the Watertown Central Registration Form prior to beginning school.
Registration for both new and returning students in the 2022-2023 academic year is NOT complete until you prove residency.

All contact/medical information must be updated and all residency documents must be re-submitted when entering grades 3, 6, and 9 based on the district’s residency policy 5033R. State law outlines that a child is to be educated in the town in which he or she resides. It is illegal to misrepresent your place of residence for the purpose of gaining educational services. To ensure that those registering in our schools are legal residents of the Watertown Public Schools, parents in the towns of Oakville and Watertown are required to provide documentation that proves residency.

3. Health Assessment Form K-12 Form | PreK Form
Who should fill out this form?
- ALL new students AND
- Returning students entering PK, Kindergarten, grades 6 and 10.
- Any student at Swift or WHS participating in an Athletics team must have an updated physical form signed by an approved medical physician.
Where do I send my Health Assessment form?
Drop off in person at your child’s school - please check your school’s summer hours to see when the main office will be open
